Application for Water/Sewer Service	12/1/2007
City Code requires that anyone wanting to turn on city water or sewer service in their name must complete an application and pay a deposit of $75.00 for water and $50.00 for sewer. Print out this form, complete, sign, and bring to City Hall at 252 SW Cedar Street to start service.
